Bronx Health Collective is a Federally Qualified Health Center affiliated with Montefiore Medical Center, serving thousands of marginalized New Yorkers in the South Bronx and across New York City. Established in 1987, the organization operates three community health center sites, provides care in clinics located within homeless shelters, and runs mobile clinics across four boroughs. The center is the flagship site of the Children's Health Fund.
The organization addresses both medical and social needs of patients who experience disproportionately high rates of chronic disease. Beyond traditional clinical care, the center recognizes that many patients struggle with food insecurity and inability to afford basic necessities, and provides comprehensive supportive services to address these economic and social barriers to health.
Services provided include:
• Chronic disease management for conditions including diabetes, asthma, and hypertension • Case management and crisis intervention • Substance abuse prevention and treatment • Nutrition services • Specialty care referrals • Insurance enrollment assistance • Transportation assistance • On-site food pantry providing shelf-stable food, baby supplies, cleaning supplies, and personal hygiene items • Educational resources for families with children
